# Morning Brief — Friday, August 28, 2026

CMS proposes work RVU cuts of 16.6% to 20.8% on primary hip, knee and shoulder arthroplasty in the CY2027 fee schedule · AlloPort opens an allograft procurement network Sept 1, leaving two platforms competing to disintermediate tissue-bank ordering · ACDF matches laminoplasty on neurological recovery in cervical myelopathy with spinal cord-canal mismatch

## Summary

3 stories this morning, for surgeons and the people who support them.

- **CMS proposes work RVU cuts of 16.6% to 20.8% on primary hip, knee and shoulder arthroplasty in the CY2027 fee schedule** — The CY2027 PFS proposed rule (CMS-1848-P) revalues primary arthroplasty: THA 27130 19.11 to 15.37 wRVU (-19.6%), TKA 27447 -16.6%, TSA 23472 -19.0%, shoulder hemiarthroplasty 23470 -20.8%. Orthopaedic surgery aggregate impact -7% (-8% facility, -5% non-facility). CMS cites a site-of-service anomaly: >50% outpatient but still valued with 90-day globals. AAHKS opposes, coordinating with AAOS. Comments close Sept 14, 2026. [Source](https://www.aahks.org/cms-proposes-arthroplasty-cuts-in-2027-fee-schedule/)

- **AlloPort opens an allograft procurement network Sept 1, leaving two platforms competing to disintermediate tissue-bank ordering** — AlloPort goes public September 1, 2026 ahead of the AATB annual conference in San Francisco, connecting accredited non-profit and commercial tissue banks directly to hospital supply chains and ASCs with real-time inventory for structural bone, osteochondral and soft-tissue tendon allografts. Competitor AlloDirect already runs a direct-to-tissue-bank marketplace and has opened it to hospitals and IDNs nationwide. - **ACDF matches laminoplasty on neurological recovery in cervical myelopathy with spinal cord-canal mismatch** — Retrospective cohort published August 14, 2026 in Clinical Spine Surgery: 323 ACDF vs 244 laminoplasty, operated 2014-2020, minimum 2-year follow-up. Comparable neurological recovery; laminoplasty showed lower adjacent segment disease risk; ACDF comparable where decompression was sufficient. Authors conclude SCCM alone should not contraindicate an anterior approach. [Source](https://www.ovid.com/jnls/jspinaldisorders)
